Third-Party Access to Medical Information

For confidentiality and data protection reasons, our practice can only discuss a patient’s medical information directly with the patient, unless we have received the patient’s consent for someone else to act on their behalf.

If you would like a family member, friend, carer, or other representative to speak to the practice about your medical records, test results, appointments, or healthcare concerns, you must first complete our Consent Form.

The completed form gives your chosen representative permission to discuss your healthcare with the practice and, where applicable, access your medical records. To protect patient confidentiality, we cannot share information with a third party until the consent process has been completed and verified.

Once we receive the form, a member of the practice team may contact the patient directly to confirm consent before access is granted.

Please complete and return the consent form to the practice. If you wish to withdraw consent at any time, please contact us and notify the practice in writing.
