Online Access to Your Medical Records
Patients at The Tutbury Practice can securely access a range of online services through approved NHS systems, such as the NHS App and Patient Access. These services allow you to:
- Request repeat prescriptions
- View and manage appointments
- Access parts of your medical record, including allergies, immunisations, test results, consultations and correspondence
Online access is available to patients aged 16 and over. Most patients can register through the NHS App, where identity verification will be required as part of the setup process. If you are unable to register using the NHS App, you can download a registration form here or collect one from reception. Once completed and returned with the required identification, the practice can arrange online access for you.
Parents, guardians and carers may be able to apply for proxy access on behalf of another patient, subject to consent, identity verification and appropriate safeguarding checks. An application to request this can be downloaded here or collected from the reception.
Parents and guardians can request proxy access to a child’s NHS records. When the child reaches 11 years of age, the practice will review this access. The child will receive a letter explaining their options and asking whether they wish their parent’s or guardian’s access to continue. Download a copy of the letter here.
The practice is committed to protecting patient confidentiality and will only provide access where it is safe and appropriate. Access may be restricted or withdrawn where there are safeguarding concerns or risks to patient welfare.
Before requesting access, please be aware that your records may contain medical information, test results or historical entries that you may find unexpected or difficult to understand. If you have any concerns about information in your record, please contact the practice for support.
