Meds safety week 2026
Every Report Counts
2-8 November 2026 marks Med Safety Week, an international campaign that raises awareness of the importance of reporting suspected side effects, adverse incidents, and safety concerns relating to medicines and medical devices.
Experiencing a problem with a medicine or medical device?
The Medicines and Healthcare products Regulatory Agency (MHRA) encourages patients, carers and healthcare professionals to report any suspected issues through the Yellow Card Scheme.
Reporting concerns helps to improve patient safety and provides valuable information that can identify emerging safety issues, improve product information, and protect others from potential harm.
What can be reported?
The Yellow Card Scheme allows you to report:
- Suspected side effects from medicines
- Adverse incidents involving medical devices
- Safety concerns relating to e-cigarettes and vaping products
- Defective, low-quality or suspected falsified (fake) healthcare products
You do not need to be certain that a medicine or device caused the problem. If you suspect there may be a link, your report could provide important information that contributes to ongoing safety monitoring.
Why is reporting important?
Every report submitted to the Yellow Card Scheme helps regulators build a clearer picture of how medicines and medical devices perform in real-world use. Even a single report may help identify patterns that lead to safety investigations, updated guidance, or improvements in patient care.
By taking a few minutes to report a concern, you can help make healthcare products safer for everyone.
How to submit a Yellow Card report
Reports can be made quickly and easily online or through the Yellow Card mobile app.
